Registration for the 2011-2012 School Year
Applications for Free/Reduced Lunches will be available at registration.
Additonal forms may be required at time of registration.
One set of forms must be completed for each child being registered.
Physical Examinations and proof of immunizations are needed for:
Kindergarten
6th Grade
9th Grade
All students new to the district.
Vision Examinations are needed for:
Kindergarten
All students new to the district.
Dental Examinations are needed for:
Kindergarten
2nd Grade
6th Grade
All students new to the district.
Your child (grades K, 6, 9, and all new students) must have an Illinois School Physical and all immunizations up-to-date and on file with the school office by the first day of school or your child will not be able to attend.

Completing the registration forms requires Adobe Reader. If you receive an error opening the files, check to make sure that you have the correct software installed.
