PROVIDE TWO PROOFS OF RESIDENCY & BIRTH CERTIFICATE
(COPIES TO SCHOOL SECRETARY)
STEP 3:
COMPLETE PARENT AUTHORIZATION FOR RELEASE OF SCHOOL RECORDS
(RECEIVED FROM SCHOOL SECRETARY)
STEP 4:
COMPLETE NEW FAMILY REGISTRATION REQUEST IN TEACHER EASE

RECEIVE REGISTRATION CONFIRMATION EMAIL & TEACHER EASE LOGIN
STEP 6:
LOGIN TO TEACHER EASE & COMPLETE ONLINE REGISTRATION
STEP 7:
REVIEW DISTRICT POLICIES
STEP 8:
SIGN & RETURN COMPLETED REGISTRATION & POLICY AUTHORIZATION FORMS
STEP 9:
COMPLETE & RETURN STATE REQUIRED HEALTH FORMS
STEP 10:
PAY REGISTRATION FEES
(FREE/REDUCED LUNCH APPLICATION AVAILABLE)
STEP 11:
MEET WITH COUNSELOR / PRINCIPAL FOR SCHOOL TOUR AND SCHEDULING / CLASSROOM PLACEMENT

Application and Required Forms
Applications for Free/Reduced Lunches is available during on-line registration.
Additional forms may be required at time of registration.
One set of forms must be completed for each child being registered.
Physical Examinations and proof of immunizations are needed for: Kindergarten 6th Grade 9th Grade All students new to the district.
Vision Examinations are needed for: Kindergarten All students new to the district.
Dental Examinations are needed for: Kindergarten 2nd Grade 6th Grade 9th Grade All students new to the district.
Your child (grades K, 6, 9, and all new students) must have an Illinois School Physical and all immunizations up-to-date and on file with the school office by the first day of school or your child will not be able to attend.
